Acne Agminata - A case report with review of literature.

ABSTRACT A 35 year old married female from Karachi presented at the Department of Dermatology, Liaquat National Hospital in March 2002. According to the patient, she was well seven months ago, when she developed redbrown papules on the forehead, eyelids and cheeks. The lesions were asymptomatic. The past and family history was insignificant. Physical examination of the patient revealed multiple, small, symmetrical, monomorphic, smooth surfaced red-brown 1-3 mm papules on the forehead, cheeks and upper eyelids. Rest of the physi- cal examination was normal and no abnormality was detected in any other system. Sections from the punch biopsy of the skin lesion on the forehead revealed focal ulceration of the epidermis. Dermis showed multiple non caseating, well demarcated epithelioid cell granulomas (arrow heads) with Langerhans`s type giant cells (arrows). Moderate degree of lymphocytic infiltrate was seen around the granulomatous areas and there were few foci of acute inflammation and necrosis. ACE was normal. Mantoux test was negative, ESR and X ray chest were normal. Diagnosis of acne agminata was made. The lesions responded to a short course of prednisolone but recurred two weeks after tapering the steroid.
