Erysipeloid Cutaneous Leishmaniasis (A rare presentation).

A 48 years old, otherwise healthy male, resident of Palandari (Azad Kashmir) presented with four months history of an erythematous, infiltrated, relatively painless and non itchy plaque involving almost whole of his right cheek (Fig-1). The lesion started as a small boil and attained considerable size during a month. He was initially treated with antibiotics and then with antituberculous drugs during last three months by various physicians but without any improvement. Considering asymptomatic nature of the lesion, his residential background and unresponsiveness to previous treatments, he was clinically suspected as a case of Cutaneous Leishmaniasis. Skin slit smear was found positive for amastigote form of Leishmania i.e. Leishmania Tropica (LT) bodies. Skin biopsy also revealed the diagnosis of CL. He was treated with Inj. Meglumine antimonate (Glucantime) 15mg/kg-body wt. After two weeks of treatment he showed considerable improvement with marked reduction in erythema and swelling.
