Incidence of Intestinal Metaplasia in patients with Helicobacter Positive Gastritis.

Background: To determine the incidence of intestinal metaplasia in patients with Helicobacter pylori positive gastritis. Methods: The study was conducted in the Department of Pathology, in collaboration with Department of Medicine, Khan Research Laboratory (KRL) Hospital, Islamabad, from July 2000 to December 2001. A total of 50 patients (26 males and 24 females) with H. pylori positive gastritis were studied. All patients underwent upper G.I. Endoscopy. Biopsies were obtained from antral region as well as the body of stomach. Alcian Blue staining of tissue slides was done. Evidence of intestinal metaplasia (both complete and incomplete type) was sought and recorded, if present. Result: Out of the 50 patients studied, six (12%) showed intestinal metaplasia. Of these, four showed incomplete and two complete intestinal metaplasia. There was only one female among the six positive cases. She had complete intestinal metaplasia. The remaining five patients were males, one of whom had complete and four incomplete intestinal metaplasia. Conclusion: H. pylori gastritis is associated with incomplete and complete intestinal metaplasia. A close follow up with repeated endoscopy and biopsy is suggested in such cases because there is increased risk of malignancy.
