Massive Neurofibroma of the sciatic nerve.

A ten year old child presented with a gross painless swelling of the right buttock and right scrotum, causing deviation of the penis to the left size. The swelling had an irregular surface and was mobile on the transverse axis, but not longitudinally. The patient also had cafe au lait spots and wide area of pigmentation on the lateral side of the right buttock overlying the tumour. There was no neurological deficit, and functions of the afflicted limb were normal. MRI examination of the swelling showed that the swelling was composed of neurofibromata extending from the nerve root of the sciatic nerve upto the popliteal fossa. During the operation, the mass was exposed. The nerve was embedded in multiple fusiform neurofibromata of white colour, giving the appearance of a thick rope composed of macaroni like neurofibromatous strands with irregular surface of 2 to 5 cm length. The whole length of the nerve was affected, from the buttocks to the popliteal fossa. Fortunately the tumours were loosely adherent to the nerve and were easily removed. The patient made an uneventful recovery, and there were no untoward complications. There was no foot drop or any other evidence of nerve damage post operatively. 155g of tumour tissue was removed, and was sent for histology. It was reported to be neurofibroma.
