Bacterial Meningitis: Pathogenesis and use of steroids.

This is a review article. Studies in humans and animals have increased our understanding of molecular pathophysiology of bacterial meningitis. These studies have prompted the evaluation of new treatments in addition to antibiotics. It has been demonstrated that bacterial components stimulate the local production of inflammatory mediators in the CSF leading to inflammation and alterations in cerebral microvasculature. Anti-inflammatory agents like glucocorticoid reduce the production of inflammatory mediators and severity of inflammatory process in the CSF and neurological sequelae. However, their beneficial effects and routine use in clinical practice remain questionable. It is expected that in future new anti-inflammatory agents and monoclonal antibodies may play important role in the treatment of bacterial meningitis with antibiotics.
