Drug resistance in enteric fever. A report from Civil Hospital, Karachi.

We report a retrospective study of 76 children admitted to the Paediatric Department of Civil Hospital, Karachi with enteric fever over two years 1992-93. Resistant strains of salmonella typhii were 26% while 64% were sensitive to amoxicillin and chloramphenicol. Treatment of these children with "first" and "second" line antibiotics was also considered. The majority of patients treated (69.73%) responded to either one of the two first line antibiotics. There was a high rate of negative blood cultures, probably due to inappropriate antibiotic use before hospitalisation. It is therefore, recommended, that children with suspected enteric fever should have a blood culture done before initiating antibiotic therapy. First line antibiotics should be used in all children unless indicated by blood culture sensitivity and/or lack of therapeutic response.
