Role of Lung Ultrasound in Detection of Respiratory Diseases in Pediatric Intensive Care Unit of a Tertiary Care Hospital of Sindh, Pakistan.

Objective: To determine the diagnostic accuracy of lung ultrasonography in detection of respiratory diseases in PICU keeping Chest X ray as gold standard. Study Design: Descriptive cross-sectional study. Place and Duration of Study: Study was conducted in the PICU of Indus Hospital, Karachi from 1st June 2022 to 31th December 2022. Material and Methods: 160 children of either gender between ages of 1 month to 12 years, fulfilling the inclusion and exclusion criteria, were included in the study. Detailed history and physical examination were carried out that was followed by point-of-care (POC) lung ultrasound for the diagnosis of different lung diseases. Portable Chest X ray in PICU was subsequently done of all these patients. Relevant data was recorded in specially designed forms that were kept in the department. SPSS version 23 was used for data analysis. Frequency and percentages were calculated for all the categorical variables while mean and standard deviation were calculated for the quantitative variables. Sensitivity (ST), specificity (SP), positive predictive value (PPV) and negative predictive value (NPV) were calculated. Results: Lung ultrasonography showed a diagnostic accuracy of 93.51% for pneumonia, 63.63 % for pneumothorax, 91.93% for pleural effusions and 80% for interstitial lung diseases. Conclusion: Our results indicated that POC lung ultrasound is a reliable, safe, radiation-free and cost-effective imaging modality in detection of common respiratory diseases and management of critically ill pediatric patients in PICU.
