Acute Appendicitis: pathological discriminating diameter.

We examined appendiceal diameter in 100 cases for determination of the suitable gross parameters for a dependable diagnosis of acute appendicitis. The maximum total width (along with mesoappendix), and net maximum gross diameter of only appendix itself were correlated with the histopathological presence or absence of the acute appendicitis. Our findings confirm that based on the maximum net gross diameter alone, a diagnosis of acute appendicitis could be achieved with great accuracy. However our findings suggest that a cut line diameter of > 0.66 mm as opposed to 0.60 nun is optimum for decision-making. This combined with clinical examination and laboratory tests could significantly reduce the numbers of unwarranted appendectomies.
