Sensitive rapid Troponin-T assay at admission point of care in predicting outcome of patients with Unstable Angina (a pilot study).

We conducted this pilot study to demonstrate effective use of rapid assay TnT. The study population consisted of consecutive patients admitted to the coronary care unit of the Aga Khan University Hospital with diagnosis of Unstable Angina (UA). The patients with definite myocardial infarction (MI) at the time of presentation were not included. All patients had their troponin estimated at time of admission in emergency (ER) using a Brohinger sensitive rapid assay hand held device (cut off 0.1 mg/dl). If first TnT test was negative 2nd test was repeated after 4 hours before calling it a negative test. Other hematological assessments including Creatinine kinase (Ck) and CkMB were done according to the protocol with minimum of three estimation 7 hours apart. RESULTS: Out of 196 patients these, 36 patients with UA who consented were included in the study. These 35 patients were followed for 10 weeks (70 days) after the discharge from Hospital. 15 42-85%) patients were positive for TnT and 20 (57%) were negative (table 3). Baseline demographic values in the Troponin T positive and troponin T negative patients showed very little difference - clinically between the groups except Non-QMI at the time of admission, seen in 6 patient, all in the positive group 6/15 (40%, P = 0.0041). Multivariate analysis to evaluate independent prognostic value of TnT in comparison of Ck or ECG was not performed due to small sample size.
