Treatment of Intertrochanteric fracture in adults with Dynamic Hip Screw without Radiological control.

A prospective study of 60 patients with intertrochanteric fractures treated with DHS without X-ray control was carried out at Orthopaedic Department, Services Hospital, Lahore between January 2000 to November 2000. The objective was to assess the final out come in terms of failure rate which included implant failure, nonunion, infection, avascular necrosis and reoperation. A.O. classification of intertrochanic fractures was used as a parameter of stable and unstable fractures. There were 22 (44%) stable fractures and 28 (56%) were unstable fractures. Results show 20 out of 48 (40%) patients had ideally placed implants. 22 out of 48 (44%) patients less ideally placed implants. Six patients (12.4%) had non-ideal placed implants. In the end we conclude that D.H.S. osteosynthesis in introchanic fractures treated without radiological control by an experienced surgeon and good preoperative planning is an effective alternative method of fixation of such fractures at places where image intensifier / portable X-ray facilities are not available.
