Pattern of upper Femoral fracture in a tertiary care hospital of Karachi.

Aim of Study: A retrospective analysis was done of all the upper femoral fractures presenting to Abbasi Shaheed Hospital Orthopaedic Unit II to find out the pattern of upper femoral fractures to see whether the pattern is similar to the western world or different. Study Design: Descriptive study. Material and Methods: All upper femoral fractures admitted to Unit II Orthopaedic Department from 1993 to 2001 were analyzed by viewing the admission record register. Results: A total of 620 patients with upper femoral fractures were admitted. Out of these, 50% were Intertrochanteric fractures (n=312), 40% were neck of femur fractures (n=246) & 10% were subtrochanteric fractures (n=62). Majority of fractures were in the age group of above 40 years. Surprisingly, males were more in all age groups as compared to females in all three types of fractures. Fractures in younger age group were due to fall from height or road traffic accident. Intertrochanteric fractures were more as compared to neck of femur fractures. Also for some reason, upper femoral fractures excluding subtrochanteric fractures occurred 15% more on the left side as compared to the right. Conclusion: The pattern of upper femoral fractures in our study is completely different from that reported in western literature. Both Intertrochanteric & neck of femur fractures are found more in males as compared to females. Left side fractures are more common than right side for some reason. Also, large number of fractures were in the age group of 40 years & above, which is also different from the western literature in which majority of fractures occur above 70 years of age.
