Experience with pedicled Pectoralis Major Myocutaneous Flap in soft tissue reconstructions of head and neck region.

This retrospective study was carried out in the Department of Oral and Maxillofacial Surgery, Mayo Hospital, Lahore between January 2000 and April 2003 to highlight the role of pedicled pectoralis major myocutaneous flap in reconstruction of soft tissue defects in the head and neck region. A total of 30 cases, 21 males and 9 females, post traumatic as well as post excisional were included in the study. The flap was utilized with single skin island in 18 patients and double skin islands in 12 patients, both for lining the oral cavity and skin coverage. Two of the flaps failed completely, in one case there was loss of one skin paddle out of the two, while in the other case with double skin islands, there was partial loss of one skin paddle. Rest of the flaps did well. Suture separation was noticed in three cases. Neck contracture was another complication noted in three cases. Donor site was closed primarily in all the cases with uneventful healing except in one case. Hence we conclude that this flap provides excellent single stage reconstruction option providing both the cover and oral lining simultaneously with primary closure of the donor site and least morbidity.
