Follicular cyst in a three years old child - evidence based clinical case.

A three-year-old child, accompanied by her father, was referred to my private practice complaining of swelling of lower left posterior part of the face over a period of one month. Extra oral clinical examination showed a soft swelling in the region of the left posterior part of the mandible. The swelling was not tender to palpation. The remaining part of the mandible was normal. Intra orally there was large buccal swelling on left side of the mandible extending from middle of the mandibular body to the mid ramus region. The general condition of the patient was good, the full Complement of deciduous teeth being present. None of the teeth were carious or tender to percussion. An OPG showed a large cystic lesion involving the distal root of the lower left second deciduous molar and extending up to the upper half of the ramus. Within the cystic lesion the lower left first permanent molar was displaced near the lower border of the mandible. Aspiration was performed with a broad bore needle and about 20cc of straw colored fluid was aspirated. After routine investigation, the child was operated under general anesthesia. A large cyst was enucleated together with lower left first permanent molar and extraction of lower left second deciduous molar. The inferior alveolar nerve was identified and preserved. After toilet of the cavity, the wound was closed primarily. The cyst lining was send for biopsy which confirmed the diagnosis of follicular cyst. Protein estimation was also more than 4gm/100ml, which is typical of follicular cyst.
