Oesophageal Varices vs Peptic Ulcer: A Study of 100 Patients Presenting in Mayo Hospital with Upper Gastrointestinal Bleeding.

The study was carried out to compare the incidence of oesophageal varices with peptic ulcer. 100 patients of UGI bleed were included. Fifty four had variceal bleeding among which 44% were HBsAg positive, 19% had Hepatitis C and 7% had both. Out of these 96% had cirrhosis and 4% had non-cirrhotic portal hypertension. Thirty one patients of variceal bleed had jaundice. Thirty one among variceal bleeding were in Child`s Class C, 19 in Class B and 2 in Class A. Four patients had both varices and peptic ulcer, 2 had Mallory weiss tears, one had Arterio-Venous malformations and no cause found in 6 patients. Peptic ulcer disease was present in 32 patients, in which 34% were on NSAID. Sixty eight patients had haematemesis and melaena, 19 had haematemesis and 13 had melaena. Eight patients died, 6 with variceal bleeding, and one had perforated duodenal ulcer. Uncontrolled bleeding from varices was seen in 9 patients. It is concluded that incidence of oesophageal varices was double as compared with peptic ulcer.
