Long-term consequences of live-related renal donation.

Background: In the United States of America, in 2001 for the first time, more living donors kidneys than cadaveric kidneys were transplanted. In Pakistan we are yet to see the cadaveric transplantation. Despite the rapid increase in live-related kidney donation throughout the world, the risk involved in uninephrectomy should not be neglected. With the objective to study the long-term consequences of live related renal donation on donors, this study was conducted. Research Methodology: This study was conducted at Nephrology Department of PGMI, Lady Reading Hospital, Peshawar, over a period from January 1998 to December 2004. Live, related donors of patients aged 20-50 years, maintained on maintenance hemodialysis since one year and were otherwise suitable candidates for renal transplantation were included in the study. The donors` record was maintained before and after kidney donation for 7 years to study the impact of live-related renal donation on donors. A total of 23 live-related kidney donors were studied. The study was started from pre-operative assessment to 7 years post-nephrectomy. Results: Fortunately donors` post-nephrectomy infection and bleeding was seen in no patient. As such no significant decrease in glomerular filtration rate (GFR) immediately after nephrectomy occurred and during 7 years follow-up non of the live-related kidney donor ended up in end stage renal failure. After age adjustment, increase of blood pressure is noted in only one case, who had high normal blood pressure before kidney donation and no cause of hypertension could be found out after intensive investigations. Conclusion: The benefits of live-related renal transplant surpasses by far the potential risks. If careful work-up of live donor is done, short-term and long-term risks are minimal to none with an excellent outcome of live-related kidney donation.
