Frequency of Carcinoma Gallbladder in patients undergoing surgery for chronic Cholecystitis with Cholelithiasis.

Background: Gallbladder malignancy is the most common malignancy of biliary tract. Symptoms of gallbladder malignancy is usually masked by of chronic cholecystitis and therefore most of the patients present in advanced stage. This study was conducted to see the frequency of cancer of gall bladder in patients undergoing surgery for chronic cholecystitis with cholelithiasis in different age and sex groups in our population. Research Methodology: An observational study of patients admitted to department of surgery (Surgical C Unit) PGMI Lady Reading Hospital Peshawar with the diagnosis of chronic cholecystitis with cholelithiasis for cholecystectomy was carried out over a period of 2 years from 1.7.2003 to 30.06.2005. Demographics, clinical presentation, imaging studies, operative, intervention and histopathological results were examined. Results: Over the period of 2 years 364 patients were admitted with the diagnosis of chronic cholecystitis with cholelithiasis for cholecystectomy. 22 (6.04%) patients out of 364 admitted cases were diagnosed as gallbladder malignancy. Age ranged from 34 to 73 years with mean age of 55 years. Female to male ratio was 2:1. The most common presenting complaint was pain right hypochondrium (81.8%), nausea and vomiting (77.3%). Ultrasonographic examination picked up 3 patients (13.6%) with gallbladder mass and 15 with gallbladder wall thickening (68.1 %). Amongst these 22 patients with gallbladder malignancy 3 patients (13.6%) had inoperable growth. Histopathological examination reported adencarcinoma in all cases. Conclusions: Gallbladder malignancy occurs commonly in 5th decade of life. The patients should be encouraged to have gallbladder surgery once diagnosed for gallbladder disease and every gallbladder specimen should be sent for histopathological examination.
