Factors delaying the management leads to Obstructed Labor 100 cases survey in Khyber Teaching Hospital, Peshawar.

Background: Obstructed labour is a serious condition/emergency in obstetrics endangering the life of the mother and foetus, increasing the rate of perinatal/maternal morbidity and mortality. The study was conducted to find out the factors, to give attention to the problems delaying the management leading to obstructed labour other than obstetrical causes in obstetrics and to give possible recommendations. Method Method: The study was conducted in the department of Obstetrics and Gynecology, Khyber Teaching Hospital Peshawar Pakistan over a period of seven and half months starting from January 1st, 2004 to August 16th 2004 on one hundred patients admitted as cases of obstructed labour. All cases were recorded, interviewed, managed in the department, using a protocol prepared for the study on predesigned proforma. Results: 55% of the cases suffered obstructed labour because of negligence of medical staff in periphery who keep the patient in hope of normal vaginal delivery. Second most important factors was the poor condition of the roads, third contributing factor was the lack of medical facilities. Conveyance and familial taboos further add to the miseries. Conclusion: There is a lack of obstetrical knowledge in the medical staff doctors, nurses, LHVs, Dais, and TBAs in the periphery, Government hospitals (Primary and Secondary Level) and Private Sector. In addition to late diagnosis of obstetrical causes there are other factors like poor antenatal care, poor medical facilities, family taboos, illiteracy, socio economic situation, roads, transport, fear of operation, security/safety, which make the labour obstructed, and increases the perinatal/maternal morbidity mortality rate.
