Therapy of Helicobacter pylori infection in gastritis and peptic ulcers.

Objective: To evaluate the efficacy of drug regimens, given to treat gastritis and peptic ulcers associated with Helicobacter pylori infection. Design & Duration: Therapeutic drug trial conducted over a period of two years. Setting: Outpatient Department of Lyari General Hospital, Karachi. Patients: Patients of either sex above the age of 12 years with clinical symptoms of gastritis or peptic ulcer proved by positive histology, culture, biopsy urease test and serological results. Methodology: A detailed history was taken and complete physical examination was carried out in all the cases. Eighty patients who fulfilled the inclusion criteria were divided into three groups: Group-I (27) received Oral Omeprazole 40 mg in the morning plus Oral Clarithromycin 500 mg tid, for two weeks. Group-II (27) received Oral Omeprazole 20 mg bid plus oral Clarithromycin 250 mg bid and oral Metronidazole 500 mg bid for one week. Group-III (26) received oral Bismuth Subsalicylate 2 tabs qid plus oral Amoxicillin 500 mg qid and oral Metronidazole 250 mg tid for two weeks. All patients were hospitalized for 24 hours for the purpose of observation and relevant sample collection. Patients sub- sequently attended the clinic for assessment at the date of completion of treatment i.e. at 07 days and 14 days after start of treatment and then at 28 days, 4, 8, and 12 months after completion of treatment. Results: The overall clinical and bacteriological cure rate was 81% in Group-I patients, and 100% in both Group- II and Group-III patients. The side effects caused by the drugs used in all the groups were milder in nature. Conclusion: Drugs used in Group-II and III have excellent cure rates. Those in Group-II are specially commendable as they act in a short span of time and are thus cost effective.
