Role of Octreotide in Management of Post Operative Gastrointestinal and Pancreatic Fistulae.

It is a prospective study of 20 patients with 21 postoperative gastrointestinal and pancreatic fistulae. Ten patients had small intestinal, 3 duodenal, 3 caecal and 5 had pancreatic fistulae. One patient initially having pancreatic fistula later on developed duodenal fistula as well. He is the only fatal case in the study 15% mortality). In the small intestinal group, the closure rate was 80% and mean time was 17 days. Two patients required operative closure because of inability to properly care for the wound. In the duodenal group closure rate in 2 surviving patients was 100% and mean time for closure was 8 days. In the pancreatic group the closure rate in surviving patients was 100% and mean closure time was 9 days. In the caecal group closure rate was 100% and mean time for closure was 19 days. The study showed that closure of fistulae was directly related to the amount of fistula output and Irving & Beadle classification of wound. The study showed that octreotide was very effective in decreasing the fistula output. This made the care of patient with intestinal or pancreatic fistula easy, expedited the fistula closure time and probably increased the fistula closure rate.
