Prevention of postoperative shivering by prophylactic ketamine in patients undergoing major surgeries under General Anesthesia.

Objective: The purpose of this study is to determine the effect of prophylactic ketamine in preventing postoperative shivering. Study Design: Prospective comparative study of 100 cases. Place and Duration of Study: The study was carried out at the Anesthesia Department of Hayatabad Medical Complex, Peshawar from June 2003 to August 2004. Subject and Methods: One hundred patients (ASA I-II) scheduled for elective orthopedic, gynecological and abdominal surgeries were included in the study. The patients were divided into two groups of 50 patients each. One group was given prophylactic ketamine 25mg I.V, 30 minutes before the end of surgery while the other group was given Normal saline I.V. Anesthesia was induced with thiopental 4 mg kg-1, Toradol 0.5mg kg-1, and vecuronium 0.1mg kg-1. Anesthesia was maintained with halothane 0.7% and nitrous oxide 60% in oxygen. Axillary temperature was measured immediately after induction of Anesthesia, 30 min after induction and before administration of the study drug. Using a three point scale, postoperative shivering was assessed in the recovery room. Results: The number of patients shivering on arrival in the recovery room and at 10 and 20 min after operation were significantly less in ketamine group than in Normal saline group. Conclusion: Prophylactic ketamine is an effective drug in preventing postoperative shivering.
