Diabetic Foot Lesions Comparative Study of Management Modalities.

A prospective study comprising 80 patients with diabetic foot lesions was conducted at services hospital Lahore from 01/2000 to 02/2001 to identify best local management for diabetics. It included 50 males and 30 females with mean age of 63.2. Most of the patients were admitted through emergency. Detailed clinical assessment was made. Radiology of foot, renal profile and serial blood and urine sugar levels were the diagnostic tools. Patients were managed by good diabetic control, wound debridement, serial dressing with Hypochlorite (Eusol), Tetrachlorodecaoxide (Oxoferin), hydrocollide (duoderm) and calcium alginate. (Kaltostat). Two patients required primary amputation for infective gangrene, 5% (n=4) required Ray amputation of toes, 3.75% (n = 3) had to undergo below knee amputation and 1.25% (n = 1) had above knee amputation, 87.5% (n=70) healed with incision drainage and subsequent wound care. This study was conducted to identify results of different treatment modalities in the management of diabetic foot lesions. We conclude that early detection of lesion good wound debridment and dressing with control of hyperglycemia can save many diabetics from amputation. Modality of treatment was based on the type of lesion and the general condition of the patient.
