Marsupialization Followed by Enucleation of Dentigerous Cyst.

The case report highlights the importance of combination techniques of marsupial ization and enucleation in 17 years old boy with dentigerous cyst. The clinical and radiological findings with treatment modality are presented.

Case Report: On August 16, 2000, a 17 years old boy was referred to the department of Oral and Maxilo-facial Surgery, Punjab Dental Hospital, Lahore for evaluation and treatment of a radiolucent lesion involving two associated teeth in the right side of the mandible from second premolar to the angle region.

History: Patient complaint of vague pain in right mandibular arca and clicking sound in temporomandibular joint of the same side. He had a previous history of marsupial ization to allow the eruption of associated teeth four years back.

Clinical examination: On the first meeting, the patient seemed placid and dull. He had swelling on right side of mandible extended from body of the mandible to the angle region. On palpation there was tenderness. Intra-oral examination revealed missing posterior teeth and there was discharge of pus from molar area.

Laboratory studies: These included complete blood count, bleeding, clotting, prothombin time and urinalysis.. Swab was also taken for culture and sensitivity. All laboratory studies yielded normal findings except the culture test. Culture report showed growth of staph aureus after 24 hours of incubation at 37C. This microorganism was highly sensitivity to penicillin derivatives.

Radiographic examination: A panoramic radiograph showed multilocular radioluscency with impacted right mandibular second and third molars. First molar was missing and there was displacement of second premolar root distally. The radioluscency was well demarcated with radio opaque margins. Provisional diagnosis established was recurrence of cyst.
