Breast Abscess and Acute Cholangitis: Two Rare Manifestations of Salmonella typhi in the same patient.

A middle aged woman with a past history of ulcerative colitis presented with acute onset of high grade fever, shaking chills, loose stools and abdominal pain, followed by right breast pain. At presentation she was febrile, jaundiced and tender in the right upper quadrant of the abdomen. She also had evidence of a breast abscess. There was laboratory support for sepsis and biliary obstruction. A clinical diagnosis of cholangitis was made, based on Charcot’s Triad of obstructive jaundice, right upper quadrant pain and fever with chills. Salmonella typhi, sensitive to cephalosporins was isolated from both blood and breast abscess. The patient recovered completely with intravenous antibiotics and drainage of the breast abscess. This was an unusual case of S typhi cholangitis and breast abscess in the same patient.
