A 32 year old man presented to the Chest Clinic with fever and cough with significant weight loss over two months. Initially the fever was low grade specifically at the late hours of the day with malaise but then progressed to almost continuous fever and weakness that disabled him from work. The patient denied drug abuse or extramarital sexual activity. He was not aware of contact with any person with TB. On examination, he was an age-appropriate male with significant weakness requiring assistance for simple activities. He was febrile, emaciated but with non rewarding physical examination.
