Pattern of club foot referrals in public sector teaching hospital and their management.

Objective: To determine the pattern of club foot referrals in a public sector teaching hospital, and does late referrals affects management and out come. Study Design: A prospective descriptive study from 1st June 1998- 31st May 2004. Setting: This study was conducted in the department of Orthopaedics Unit- I. CHK, Dow University of Health Sciences. Subject and Methods: 450 club feet of 360 patients catered in this study. The patients who presented after 4 weeks of delivery with or without treatment were labelled as late or neglected cases. They were managed according to Somppi grading conservatively by serial casting or operatively by Turcos procedure. Results: Out of 360 patients there were 288 males and 72 females. 90 patients were referred from indoor Gynae/Obs wards. 270 were catered from out patient department. Out of 270, 130 were late referrals or neglected cases. 60 patients of neglected cases received some kind of treatment, conservative or operative at different places. 70 patients who were late referrals did not receive any treatment due to different reasons. Cases which were primarily treated had excellent results comparing to late referrals or neglected cases. Conclusion: Significant number of late or neglected cases and their difficult management is alarming. This could be avoided by nation wide screening and their treatment by trained persons at the community level.
