Two stage fixation of fracture shaft femur in polytrauma patients.

Objective: To asses the results of I/M nailing after External fixator in poly trauma cases. Material & Method: Study & Design: The study conducted from May 2006 to September 2008. Setting & Instruments: Department of Orthopaedics Unit II, Dow University of Health Sciences & Civil Hospital, Karachi. Poly trauma patients with fracture shaft emur were included into the study 19 were having open fracture of shaft of femur with other injuries including head injury, multiple fractures, abdominal injuries, chest injuries and vascular injuries , while 40 having close fracture shaft femur with other injuries mentioned above. In all cases external fixator applied for fracture shaft femur. Results: Among 59 patients 48 were male and 11 female, with age varying from 15 to 71 years average age of the patients were 34.5 years, the patients were evaluated clinically and radiologically. We were able to complete the follow up of 58 patients. Satisfactory union achieved in 92% patients who underwent intramedullary nailing. 5% patients underwent with non-union i.e. 02 cases, while joint stiffness was found in 10 patients. 7.6% patients had pin tract infection. MUA done in 02 patients, 02 went re-insertion of screws because of breakage and non-union. Conclusion: We concluded that immediate external fixator followed by early close I/M nailing is a safe treatment method for fracture shaft of the femur in selected multiple injured patients.
