Frequency of adhesion obstruction following prior laparotomy: an experience at Lady Reading Hospital.

Objectives: The aim of this study was to know the frequency of postoperative intestinal adhesion obstruction. Material and Methods: This descriptive type of study included 200 patients with intestinal obstruction having previous history of abdominal surgery, which presented to Surgical ‘B’ unit Postgraduate Medical Institute, Lady Reading Hospital Peshawar in the period between January 2007 to January 2008. Patients under 12 years of age, those with no previous history of surgery, and those managed conservatively were excluded. Data was analysed using SSPS 11. Results: one hundred patients with 69 (69%) male and 31 (31%) female were inlucded. Majority of patients i.e. 45 (45%). Eighty-two (82%) patients had undergone surgery more than 1 year ago. Indications for previous laparotomy included firearm injury (15%), appendisectomy (12%), tuberculous perforation (10%), gynaecological surgeries (9%), enteric perforation (7%), sigmoid volvulus, (7%) carcinoma colon, (6%) perforated duodenum, (5%) cholecystectomy, (5%) liver abscess, (4%) stab wounds, (4%) inguinal hernia, (4%) umbilical and/or paraumbilical hernia (3%). The record regarding cause of first operation was not available in 9 patients. In 81 (81%) patients adhesiolysis was performed, in 15 (15%) patients ileostomy and in 4 (4%) patients jejunostomy was performed. Eight (8%) patients died. Postoperative complications occurred in 36 (39.13%) patients. Conclusion: Adhesiolysis is important procedure to help patients with intestinal obstruction secondary to adhesions,not responding to conservative treatment.
