Efficacy and safety of iron sucrose complex in anemic women going through gynecological surgery and pregnancy.

Objective: To assess the efficacy and safety of intravenous iron sucrose complex in iron deficiency anemia in women with elective gynecological surgery and pregnancy. Study Design: Descriptive observational study. Place and duration: Department of Obstetrics and Gynecology, Post Graduate Medical Institute, Hayatabad Medical Complex ( HMC), Peshawar from July 2007 to February 2008. Patients and Methods: All women with iron deficiency anemia, who underwent elective gynecological surgery and antenatal patients with gestational age of 16-24 weeks, were included in the study. The patients with history of allergy to parantral iron therapy, and any bleeding disorder were excluded from the study. The patients were divided into two groups i.e. groups A and B. In Group A, patients were candidates for elective gynecological surgery with indications of menorrhagia, irregular menstrual blood loss, or dysfunctional uterine bleeding. In group B, antenatal patients with gestational age of 16-24 weeks were included. Iron sucrose complex were administered by infusion on daily basis, with 2 ampoules of iron sucrose complex in 100 cc of normal saline slow infusion, administered in 3-4 settings. Deficit was calculated by employing the simple formula that, 2 ampoules of iron sucrose complex raises the Hb by 1gm/dl including 500 mg for iron stores. The target Hb level was set as 11-12gm /dl. The Hb level was assessed 3-4 weeks after therapy. Results: In group A, the mean pretreatment Hb was 7.7gm/dl which improved to 11.34gm/dl, while in group B, the mean pretreatment Hb was 7.3gm/dl which improved to 12.1gm/dl. Side effects noted included nausea, vomiting, epigastric pain, hypotention, and mild allergic reaction. Conclusion: Iron sucrose complex is highly effective and safe in treatment of iron deficiency anemia in preoperative patients and antenatal patients. Iron sucrose complex provides an effective alternative to blood transfusion.
