Pinpoint uterine rupture secondary to placenta percreta.

Placenta accreta and percreta is a rare complication of pregnancy. Very few cases of placenta percreta in pregnancy lead to a spontaneous rupture of the uterus have been reported. We report a case of spontaneous rupture of uterus secondary to placenta percreta at 30weeks’ gestation in a woman, who presented with severe abdominal pain, features of hypovolemic shock and fetal distress. She had a past history of abdominal myomectomy for fibroid uterus. The occurrence of severe abdominal pain, suspicion of haemoperitonuem and fetal compromise, necessitated an emergency caesarean section, revealing pinpoint rupture of the uterus and placental tissue protruding through it. Hysterectomy was done due to uncontrolled bleeding. This case demonstrates that in patients with a history of uterine surgery, a careful radiological examination of the uterine wall and placentation is warranted for detection of an invasive placenta.
