Tuberculosis control through health system framework application.

Tuberculosis has resurged in the late 20th century and was declared a global emergency by the WHO in 1993. It was included in the Millennium Development Goals (MDGs) in 2000 for action by the member countries. According to the WHO report on TB 2011, the global prevalence was 178/100,000 population. Pakistan is placed 4th in the world in terms of the burden of disease behind India, China and Indonesia, despite efforts by the National TB Control Program. Beside the bacterial etiology, TB has strong socioeconomic determinants like poverty, nutrition, sanitation and housing. It is associated with diseases like HIV/AIDS and diabetes mellitus. Besides these, health systems are very strong predictors of TB control in many countries. This paper reviews a health system intervention approach by proposing the WHO six building blocks framework, including community participation and inter-sectoral approaches of Primary Health Care (PHC) alongside it. The WHO health system framework in tandem with the approaches of PHC may be useful in controlling the disease in Pakistan.
