Accuracy of "risk of malignancy index" in the preoperative diagnosis of ovarian malignancy in post menopausal women.

Objectives: To determine the accuracy of “ Risk of Malignancy Index” (RMI) in pre-operative diagnosis of ovarian malignancy, in post-menopausal women, taking histopathology as gold standard. Study design: Cross sectional study. Setting & Duration: TheGynae and Obs departments of Allied/ Divisional Head quartes Hospitals, Punjab Medical College, Faisalabad. Subjects & Methods: A total of 36 patients of ovarian tumors were included in the study. Abdomino-pelvic ultrasonography and CA125 level was performed on each patient. ACCURACY OF RISK OF MALIGNANCY INDEX was determined in terms of its sensitivity, specificity, positive and negative predictive value, using histopathology as gold standard.RISK OF MALIGNANCY INDEX was based on “RMI = MxUxCA125” M = menopausal status and score 3 was given to it, U=0 (for ultrasound score of 0); U=1 (for ultrasound score of 1); U=3 (for ultrasound score of 2-5). Ultrasound scan was scored one point for each of the following characteristics: Multilocular cyst, solid areas, metastasis, ascites and bilateral lesions. Results: RMI was found to be having sensitivity of 91.3%, specificity of 76.9%, positive predictive value of 87.5% and negative predictive value of 83.3%. Conclusions: The index is a simple scoring system which can be applied directly to clinical practice and will be of value in the pre-operative assessment of the ovarian mass.
