Objective : To assess outcomes and complications of management of Gustilo type IIIC fractures. Study Design: Descriptive case series Place and Duration of Study: Department of Orthopaedics Unit I DUHS, and Multi Centrefrom June 2011 to June 2012. Mathodology: Fifteen patients with Gustilo type IIIC fractures were included in this study. Wound debridement, arterial repair, fracture stabilization (either external or internal fixation) and soft tissue coverage were carried out in all cases. Main outcome measures were complications associated with this management. These were categorized into wound infections, neurological deficits, and deformity. The SPSS version 17 was applied to the data. Results: The mean + SD age of patients was 39.87 + 14.81 years. Majority of them suffered from non-penetrating injuries (53.3%). The overall complication rate was 60% including (26.7%) wound infection, (20%) neurological deficits, and (13.3%) limb deformity. Conclusion: The mainstays of management revolve around early and effective wound debridement, arterial repair, antibiotic prophylaxis, effective fracture stabilization and soft tissue coverage. Nonetheless, this approach leads to significant morbidity in terms of wound infection, deformity, and neurological deficits.
