Capitulum fracture: result of percutaneous fixation and review of literature.

Objective: To assess outcome of Closed manipulation and K wire fixation in capitulum fractures. Study Design: This is a retrospective case series. Place and Duration of Study: This study conducted in the department of orthopaedic surgery Karachi Medical & Dental College and Abbasi shaheed hospital from 2005 to 2009. Material and Methods: Four patients with type I capitulum fracture were identified. The mean age was 29 years. The mean follow up was 11 months. Diagnosis was made on plane radiograph anteroposterior and true lateral views. All patients were operated under general anaesthesia. Under image control, closed manipulation done. Percutaneous k-wire fixation done. The stability was confirmed under image intensifier . Elbow was splinted in 900 degree flexion and neutral forearm rotation in pop back slab for 4 weeks. K-wire removed at six weeks postoperatively. The elbow was mobilized after 4 weeks of splintage. Results: All the fractures united at the eight weeks postoperative time. Two patients has normal range of motion. One patient had extension lag of 10 degree at the last followup. One patient had extension lag of 150 degree and flexion upto 1200. All the patients were satisfied with the procedure and had no functional deficit. Conclusions: Excellent results reported with no complication in this small series of patients. We felt difficulty in rehabilitation.
