Non-cemented total hip arthroplasty in industrial workers.

Objective: To determine the results of non-cemented total hip replacements in young industrial patients of poor socio- economic background. Study Design: A retrospective case series study. Place and Duration of Study: Department of Orthopaedics and Traumatology Nawaz Sharif Social Security Teaching Hospital Multan Road Lahore Pakistan from August 2005 to August 2010 Patients and Method: 33 young patients from industrial background with low socioeconomic status were selected for Non cemented THR. All patients were operated by principal Author. The non-cemented total hip replacements were hydroxylapatite coated. Majority of the cases were idiopathic avascular femoral necrosis. There were three cases of bilateral involvement of the hip joint. All the patients were assessed pre-operatively by Harris Hip score. The procedures were performed using posterior and Harding approach. Weight bearing was started at 12 to 14 weeks. Results : All the patients were assessed pre-operatively by Harris Hip score which were from 39 to 59. Average shortening was 2cm except in 3 hips (02 of girdle stone procedure and one old fracture neck of femur. Mean Harris Hip score improved from 45 to 88-90 at 1 year and 3 years follow up 60% of the patients went back to their original duty and rest were doing light jobs or working as house wives. One patient had fracture of greater trochanteric during insertion of femoral component which was fixed with circlage wire. We had one case of superficial wound infection which settled with antibiotics. There was no DVT, neurovascular damage and residual shortening. Two hips have implants subsidence and 4 had Heterotrophic Ossifications 60% patients went back to their original jobs and rests were doing light jobs where stairs climbing and weight lifting was avoided. Conclusion: Although the follow up is very short but so far non-cemented THA is a good option in younger patients with complex Hip problems. It has significantly improved the functional abilities and eliminates pain of the patients.
