Percutaneous suprapubic cystolitholapaxy in children with vesical calculi (a new minimally invasive technique).

Objective: To assess the efficacy and safety of per-cutaneous suprapubic litholapaxy (PSL) in children with vesical calculi and to compare this procedure with traditional open vesicolithotomy (OVL). Patients and Methods: Per-cutaneous suprapubic cystolitholapaxy is a newly improvised minimally invasive technique for removal of vesical calculi in children. The procedure is being described as has been practiced in our unit. From January 1998 to October 2000, all patients below the age of 12 yrs with vesical calculi were divided into two groups i.e. Group I calculi < 4.0 cm in diameter, Group II calculi > 4.0 cm in diameter. Group I is subjected to PSL while Group II is subjected to OVL. Age and sex distribution, stone size, operating time, postoperative pain and complications were compared in the two groups. Results: 14 patients were subjected to PSL (Group I) and 12 patients underwent OVL (Group II). It was seen that Group I patients showed shorter hospital stay, less morbidity and less post-operative pain while the operative time was somewhat longer. Conclusion: This innovative technique of percutaneous cystolitholapaxy has been found to be safe and effective for treating small vesical calculi without requiring any special paediatric equipment. As shown, this technique, has lower morbidity, needs lesser post-operative hospital stay and allows ,for earlier return of children to normal activity.
