Management of complex tibial plateau fractures with illizarov fixator.

Objective: To assess results of small wire external fi xation using a ligamentotaxis technique for high energy tibial plateau fractures. Methodology: From July 2008 till July 2010, 22 patients aged 18 to 50 years underwent small wire external fi xation for high energy tibial plateau fractures. 12 involved the right and 10 the left knee. 21 were closed and one was open injuries. Fractures were classifi ed according to Schatzker’s staging system. Aft er a minimal of 2 years’ follow-up (range, 24–42 months), each aff ected knee was evaluated using knee society score (KSS). Results: Th ere were 14 type-VI and eight type-V Schatzker tibial plateau fractures. Complications consisted of: one deep infection, two pin site infections, one deep venous thrombosis and one malunion. Th e mean range of knee movement was 120 degrees fl exion and six degrees lack of extension. According to KSS criteria results were excellent in 12 patients (54.54%), good in six patients (27.27%), fair in three patients (13.63%) and poor in one patient (4.54%). Conclusion: Small wire external fi xation allows anatomical reconstruction of the articular surface, stable fi xation of fracture fragments, early movement of the joint, and care of associated soft tissue injuries, without a high rate of complications.
