Role of 24 Hours Antibiotic Prophylaxis in Clean Obstetric and Gynaecological Surgery.

Objective: To find out prevalence of post operative infectious complications after 24 hours antibiotic prophylaxis in clean obstetric and gynaecological surgeries. Place of study: Pakistan Atomic Energy Commission General hospital Islamabad. Materials & Methods: This was a descriptive observational study, which included 314 Obstetric and gynaecological patients who underwent surgery in 10 months period. Patients with preexisting clinical or sub clinical infections, diabetic, anaemic and immunocompromised patients were excluded from the study & were given 5-7 days course of antibiotics after surgery. Patients were followed at one and six weeks postoperatively and assessed for development of wound infection, urinary tract infection , endometritis and bacterial vaginitis. A specially designed performa was used for collection of data. Results: Out of 314 patients who received 24 hours antibiotic prophylaxis 10(3.1%) patients developed wound infection; One had full thickness wound infection and rest 9 had superficial wound infections. 4 patients had urinary tract infection. Another 4 patients developed post operative fever lasting for more than 24 hours, 2 of them responded to antimalarials and rest 2 were treated as cases of enteric ferver by medical specialist.No patient in study group developed endometritis or bacterial vaginitis. Conclusion: 24 hours antibiotic prophylaxis does not increase the rate of surgical site infection and other infectious complications. These results justify the use of 24 hours antibiotic prophylaxis in patients undergoing clean obstetric and gynaecological surgery.
