Cataract extraction with intraocular lens (IOL) implant is the most commonly performed surgery in ophthalmology. It is not uncommon to find that the pupil is eccentric, irregular or immobile resulting in altered dynamics of intraocular pressure and affecting vision. In this study a total of 231 eyes were studied that underwent cataract extraction with posterior chamber intraocular lens implant. Amongst all the eyes 212 (91.78%) had central pupil while 19 (8.22%) had corectopia. The commonest cause was posterior synechiae which was seen in 4.32% eyes, other less common causes included entangled haptic of the IOL in the iris 1.29%, anterior synechiae 1.15% iris atrophy 1.15%, capsular tag 0.43% and pupillary capture 0.43%.
