Outcome of Three Ports Versus Four Ports Laparoscopic Cholecystectomy in Terms of Mean Operative Time, Hospital Stay and Pain.

Objective: To compare three ports versus four ports laparoscopic cholecystectomy in terms of mean operative time, hospital stay and pain. Study Design: Randomized control trial. Place and Duration: The study was carried out at surgical department of PAEC hospital, Islamabad from 22.06.2016 to 22.12.2016 Methodology: Seventy-eight patients fulfilling the inclusion criteria were included in the study. In patients coming to the surgical outpatient department, diagnosis of cholelithiasis was made on the basis of history and ultrasound abdomen showing gallstones. Patients were randomly divided into two groups i.e., group 1 (three port) and group 2 (four port) using lottery method. Surgery times and hospital stay were measured. Pain was measured by the visual analogue score at 12 hours after surgery. Data was analyzed by SPSS 20. Results: The mean age in cases treated with three and four port was 42.23±10.24 years and 44.31±10.79 years. There were 28(35.90%) male and 50(64.10%) females. The mean surgery time in three and four port groups was 40.64 ± 12.29 and 39.17±16.23 minutes. The mean pain was 3.51 ± 2.13 in three port groups and 3.76±2.20 in four port groups. The mean hospital stay was 2.13 ± 0.57 days and 2.17±0.34 days in three and four port groups respectively. We found no significant difference of mean age, and outcome in both groups, p-value > 0.05. Conclusion: There is no significant difference in outcomes such as duration of surgery, pain and hospital stay using three versus four ports. Depending on surgeon’s choice any of the procedure (3 or 4 ports) can be used.
