Maternal Outcome in Primigravid Women Induced at Term with Normal Versus High Body Mass Index.

Objective: To compare maternal outcome in primigravid women induce at term with normal versus high body mass index. Place and duration of study: Mother and Child Health (MCH) Center, Pakistan Institute of Medical Sciences (PIMS) Hospital, Islamabad from September 2012 to March 2013. Methodology: This was a prospective cohort study, which included a total of 160 nulliparous women with a singleton pregnancy. They were admitted for induction of labour at a gestational age of 37 to 41 weeks with a bishop score < 6. Women were categorized into two groups .80 patients in group A with higher BMI≥25kg/m2 and group B with normal BMI of 18-25kg/m2. Mutigravida women and BMI<18 were excluded. Maternal outcomes were judged in terms of failed induction and spontaneous vaginal delivery versus cesarean section. Results: The obstetric outcome was compared between group A (The high BMI group of ≥25) with group B (the normal BMI group of 18 to <25). The mode of delivery was cesarean section in 38 (47%) in group A compared to 23 (29%) in group B. Similarly, in group A 42 women i.e. 53% of cases had a spontaneous vertex delivery while in group B, 57 women i.e. 71% had a spontaneous vaginal delivery which was statistically significant. In group A only 5 cases had failed induction but in group B all the cases had a successful induction which was statistically significant. The mean age of the patient was 24±6 in both study groups. The average gestational age was 39±2 in both study groups and the difference between these two means among study group was not statistically significant. The average gestational age was 39±2. The bishop score was 2.3 in group A and 2.4 in group B with P-value of 0.58 which was statistically insignificant Conclusion: Our findings support the widely recognized and accepted clinical fact that obesity increases the risk of cesarean section and risk of failed induction.
