Adult Acute Lymphoblastic Leukaemia: A Clinico-haematological Study of 67 Cases.

In this study clinical features and haematological parameters have been evaluated in 67 adult patients (> 15 years) of acute lymphoblastic leukemia at the time of presentation. These include factors that are prognostically highly significant such as age and leukocyte count as well as other factors associated with prognosis which include sex, organomegaly, haemoglobin concentration and platelet count. The patients were divided into 3 age groups of decreasing prognostic favorability i) > 15-35 years ii) 35-60 years and iii) > 60 years. There were 58 (86.6%), 8 (11.9%) and 1 (1.5%) patients in these age groups respectively. Twenty nine Le. 43.3% of the total patients were between 15 and 20 years of age. There were 54 males who constituted the vast majority (80.6%) of the total cases and 13 females. Enlargement of lymphnodes teas present in 40 (59.7%), hepatomegaly in 48 (71.6%) and splenomegaly in 53 (79.1%) patients. All three organs were enlarged in 27 (40.3%) while hepato splenomegaly in the absence of lymphnode enlargement was seen in 18 (26.9%) cases. Haemoglobin was below 5g/dl in 23 (34.3%), 5-10 g/dl in 39 (58.2%) and >10 g/dl in only 5 (7.5%) patients. Total leukocyte count was subnormal (< 4*100000000/L) in 19 (28.4%) cases, in the normal range (14-11 x 109/L) in 14 (20.9%) and above normal in the rest; counts >5OxlO00000000/L being present in 16 (23.9%) cases. Thrombocytopacnia was present in 60 (89.5%) patients; platelet count was <25xlO00000000/L in 15 (22.4%), 25-50x109/L in 9 (13.4%), 50-l00x109/L in 31 (46.3%) and 100150x109/L in 5 (7.5%) patients. Periodic acid Schiff Staining was studied in 20 cases, of these 11 (55%) showed a positive reaction in blast cells.
