Comparison of the Adherence to Metered Dose Inhalers Versus Dry Powder Inhalers in patients with Newly Diagnosed Chronic Obstructive Pulmonary Disease.

Background: Chronic obstructive pulmonary disease (COPD) is a preventable and treatable disease characterized by shortness of breath and cough. Inhalation agents are used for its treatment. Objective: This study was conducted to compare the adherence to treatment of metered dose inhalers (MDIs) with dry powder inhalers (DPIs) in newly diagnosed patients with COPD. Method & Materials: This randomized controlled trial was carried out in Hayatabad Medical Complex, Peshawar at medical units for 06months. A total of 110 patients were included via non probability sampling by using proportion of MDIs adherence = 66.8%, proportion of DPIs adherence = 89.5% Power of test = 90%, Level of significance = 5%. Patient of either gender with newly diagnosed COPD and aged 45 to 60 years were included in the study. Patients using both types of inhalers, using inhalers irregularly and incorrectly, with pneumothorax, cardiac arrhythmia and pulmonary oedema were excluded. Data was analysed using SPSS-17. Mean ± SD was calculated for numerical variables, like age while frequencies and percentages for categorical variables like gender, inhaler type and adherence. Results: In group A (patients using metered dose inhalers) 42% patients had high adherence (score =2)to the prescribed inhaler, while 58%patientshad low adherence (score =3), whereas in Group B (patients using Dry powder inhalers) 71% patients had high adherence (score =2),while 29% patients had low adherence (score =3). Conclusion: Our study concludes that adherence to dry powder inhalers was better as compared to metered dose inhalers in the management of newly diagnosed patients having COPD.
