Gender, Age of the Patient and Killip Class at Presentation as Predictive Factors for the occurrence of Left Ventricular Thrombus in first Acute Anterior Myocardial Infarction.

The development of Left Ventricular Failure (LVF) and Left Ventricular Thrombus (LVT) in patients with first AMI is more often observed in elderly men. Whether a poor Killip Class grading, age and gender can be predictive of development of LVT during hospital need to be studied. We studied the occurrence of left ventricular thrombus (LVT) in first episode of Acute Q wave Anterior Myocardial Infarction in patients presenting to Mayo Hospital between Jan. to June 2000 and evaluated the predictive value of gender, age of the patient and Killip Class at presentation for LVT occurrence. A total of 30 (24 males, 6 females) patients with first episode of acute anterior wall myocardial infarction were included in this study. Average age of males Vs females was 51.79 „b13.86 years and 54.83 „b10.15 years respectively. Eight (males 6, females 2) patients (26.66%) were found to have LVT, on 2-D echocardiography on day 6. The mean age of the patients who developed LVT was 59.37 „b10.95 years, and of those not having LVT was 49.86 „b13.11 years. When compared men who developed LVT had a mean age of 62.67 „b10.19 years Vs 57.5 „b11.09 years for females with no statistical significant. In this study 11 (36.66%) patients were in cardiac failure (Killip class >1) at presentation and 19 (63.33%) were not. LVT developed in 6 (54.5%) patients having Killip class >I, and in 2 patients having Killip class 1. The mean age of patients who were in Killip class >1 was 59.8 „b13.61 years, and the mean age of patients in Killip class 1 was 48.7 „b11.44. We conclude that gender does not have any effect on the development of LVF or LVT during in-hospital stay after first AMI. LVT after AMI occurs more frequently in patients older than 50 years of age who also have a Killip class >1 at presentation. Echocardiography is a reliable method for the screening of patients for LVT and should routinely be carried out in patients with anterior AMI before discharge from the hospital, especially if they are in cardiac failure and if possible on follow-up visits as well.
