Acute Intermittent Porphyria.

A 16 years old girl was admitted in surgical ward with 02 days history of severe pain abdomen. She also developed high grade intermittent fever. Abdominal pain was sudden in onset, episodic and colicky in nature. It was diffusely distributed and there was some tenderness over the lower abdomen. She was kept in surgical ward and next day was operated for a suspicion of acute appendicitis, but her abdominal pain persisted. On the 2nd post operative day, her blood pressure rose up to 170/110mm of Hg and she developed tachycardia and sweating. She also developed epileptic fits and passed reddish brown urine. She was examined by Medical specialist and shifted to Medical ITC. Provisional diagnosis of Acute Intermittent porphyria was made, which was precipitated by general anaesthesia. She was treated accordingly.
