Change in functional outcomes before and after total knee arthroplasty: a prospective study on osteoarthritic patients.

Objective: To assess the improvement in functional outcomes of Total Knee Arthroplasty (TKR) in patients of osteoarthritis. Methodology: Male and female patients having age between 45-70 years who underwent unilateral- or bilateral TKR for the end-stage of osteoarthritis of the knee were included, between July to November 2017 in the Ghurki Trust and Teaching Hospital, Lahore. This was a 6 months follow up study with the use of the WOMAC scale. Data recording, storage, and assessment were carried out with the help of SPSS software Version 23. Statistical analysis of differences in pain, stiffness, and function score were carried out by using the paired T-test. Results: Out of 68 patients, 33 were males and 35 females. The percentage of bilateral TKA was 67 while the right knee and left knee percentages were 20.6 and 11.8. There were significant differences between pre- and post-operative WOMAC pain (p=0.00), stiffness (p= 0.00), function scores (p=0.00), and overall WOMAC scores (p<0.02). Mean six months pre- and postoperatively WOMAC pain score improved 0.97+-1.2; joint stiffness score improved 3.7+-2.02 and WOMAC function score improved -24.8+-9.1. The average of six months WOMAC score improvement was 0.88+-2.3. Conclusion: Despite significant WOMAC improvements and better clinical outcomes, patients did not achieve the level of outcomes reported for healthy subjects. Higher preoperative WOMAC scores had negatively influenced the WOMAC scores postoperatively.
