Role of prompt Endoscopy in diagnosis of dyspepsia at Combined Military Hospital, Rawalpindi.

There is a controversy about role of prompt endoscopy in investigating dyspepsia. In this study experience at Combined Military Hospital, Rawalpindi has been analyzed. Endoscopy of 200 randomly selected patients was carried out who presented with dyspepsia from June 2001 to Jan 2002 at Combined Military Hospital, Rawalpindi were subjected to prompt endoscopy. The patients belonged to two socio-economic classes, affluent and poor. In all the patients antral mucosal biopsies were taken at the time of endoscopy, which were sent to Armed Forces Institute of Pathology for histopathological diagnosis and detection of helicobacter pylori. The results were analyzed to calculate frequencies of different disease groups with reference to gender, socioeconomic class and helicobacter pylori status. Non-ulcer dyspepsia was identified as the major cause of dyspepsia comprising 89% of these patients. Duodenal ulcer comprised of 14% and carcinoma stomach only 7%. Most of the patients with Ca stomach had alarm symptoms. No patient with Ca Stomach was less than 50 years of age. Emperical Helicobacter Pylori eradication was not only cost-effective but equally safe as well, provided due attention was given to presence or absence of alarm symptoms in deciding about endoscopy.
