Decreased vision as the presenting complaint of Infective Endocarditis.

A 28 year old man presented with decreased vision in the right eye. Ocular examination revealed macular white-centered retinal hemorrhage associated with premacular hemorrhage. Echocardiogram revealed mitral valve prolapse with vegetations and dilated left ventricle. Blood cultures grew streptococcus viridans. The diagnosis of bacterial endocarditis was confirmed. After treatment with penicillin and gentamycin remarkable improvement of visual acuity was noticed.
