Risk stratification of patients presenting with first acute Myocardial Infarction with serum Cardiac Troponin-T.

CK-MB as serum marker of acute myocardial infarction (AMI) has some limitations. Cardiac troponin-T (cTnT) bridges these gaps. Its level within 24 hours after AMI are associated with increased number of cardiac events, attains 50% sensitivity within 3-4 hours and is more sensitive prognostic marker in unstable angina. We evaluated the efficacy of cTnT for inhosptial risk stratification of patients with first episode of AMI and compared its prognostic value with that of CK-MB. Thirty five patients (29 male, 6 female) with first episode of AMI, presenting within 12 hours of onset of symptoms were studied. Their blood samples were analyzed for cTnT, total CK and CK-MB. The correlation co-efficient between cTnT and other two was statistically significant (p<0.0001). Cardiac tropnin T is superior to CK-MB in risk stratification of patients with unstable angina and suspected AMI. Diagnostic sensitivity of cTnT was 60% at 4 hours. Cardiac TnT was a superior marker for predicting complications (p<0.0001), valuable tool for the assessment of risk in these patients.
