One-Year renal outcome of Pregnancy related kidney failure.

This prospective study was conducted in the department of nephrology, Islamabad Hospital in collaboration with Maternal and Child Health Center, Pakistan Institute of Medical Sciences, Islamabad, to evaluate the renal outcome of pregnancy related renal failure. Thirty one patients of established pregnancy related renal failure were inducted in this study. The mean age of the patients was 30.16 years. Out of these 31 patients, 23 (74.2%) developed renal failure in late and 8 (25.8%) in early pregnancy. Most of these women (23--74.2%) were multipara and 8 (25.8%) were primigravida. Twenty one (67.7%) pregnant women did not receive any form of antenatal care at any stage of pregnancy. In most of the women (67.7%) delivery/abortion took place or was attempted at home. IUD, hemorrhage (APH/PPH) due to pre-eclampsia / eclampsia and abortion were observed in 29 (93.5%) of the patients. Fourteen patients were anuric, 11 oliguric and 6 were non-oliguric but all had well established ARE Acute tubular necrosis (ATN) due to hypotension especially because of loss of blood, volume depletion and sepsis was present in 17 (54.8%) of patients. Complete renal recovery was observed in 19 (63.3%) patients, out of whom 6 (20%) recovered without dialysis support. Twenty four (80%) of the patients required dialysis support during their treatment. Four patients (13.3%) developed irreversible renal dysfunction. Mean stay of the patients in the hospital was 22.8 days. Mortality was 23.3% and one year survival was 76.7%.
