Comparison of Intraperitoneal Analgesia with Bupivacaine/ Bupivacaine with Ketorolac after Laparoscopic Cholecystectomy.

The purpose of this study was to compare pain relief and recovery variables of bupivacaine and ketorolac used in combination by intraperitoneal (i.p.) instillation and to compare this with i.p. bupivacaine after laparoscopic cholecystectomy. Sixty patients were sequentially allocated to one of two groups to receive either 20m1 of 0.25% bupivacaine i.p. (group 1), or 0.25% bupivacaine and 30 mg ketorolac i.p. (group 2). The i.p. Instillation was made under the surface of both domes of the diaphragm, the gall bladder bed and the dissected Callot`s triangle. VAS pain scores at rest, on movement and on coughing were measured at 3, 6 and 12 hours after operation. The time to fust demand analgesia, total analgesic requirement, and recovery variables were investigated. In Group 2, pain scores were significantly lower than group I at rest and on movement at 3 and 6 hours (p<0.05). There was no statistically significant difference of parenteral analgesia requirement between the two groups although more nalbuphine i.v. was used in the group 1. We concluded that the combination of intraperitoneal bupivacaine and ketorolac is better than bupivacaine alone in providing analgesia following laparoscopic cholecystectomy in the early post operative period.
